A big problem that exists is ads that people think are part of the application. An example is a quiz that has an ad at the bottom with 'Continue>>>' on it. This has an extremely high CTR and a lot of the times it brings a user to a page where they need to enter their cellphone number to take a quiz. User's are clearly tricked and then they end up with a $9.99/month bill, which a lot of people don't realize until several months later.

As a facebook developer who used one of the “good” ad netwoks (cubics, who has put strict advertising guidelines into place now) even I have seen my CPM payouts take a nose dive because of this. With 2 less networks, that means more advertisers have gone to use my ad network, thus running the “clean” inventory dry much faster.
